These glands are connected to a follicle.  Inside each follicle is a fine strand of hair (see image) – the combination of these items makes up a pilosebaceous unit, also called a PSU. <span id="more-111"></span>The areas of the body most densely covered with PSUs are the face, upper back and chest. The sebaceous glands produce oil called sebum that generally leaves through the follicle’s opening, known as a pore. Doctors generally refer to acne as a disease of the pilosebaceous unit.</p>
<p>Sometimes the follicle canal can become narrow making it hard for the sebum to escape through the pore. When this happens, the oil mixes with the cell lining of the follicle (keratinocytes), allowing bacteria to grow in the clogged follicle. The growth of bacteria can lead to inflammation. Once the follicle walls break down, everything trapped inside spills into the surrounding skin leading to zits or acne lesions.</p>
<p>Acne can manifest in a variety of forms. The most basic acne lesion is known as a <a href="http://www.clearcomplexionb5.com/how-does-acne-develop/acne3/" rel="attachment wp-att-129"><img class="size-medium wp-image-129 alignright" alt="Acne Diagram" src="http://www.clearcomplexionb5.com/wp-content/uploads/2013/01/acne3-300x150.jpg" width="300" height="150" /></a>comedo and is just a plugged and enlarged hair follicle.  If this clogged follicle remains below the skin’s surface it’s considered a closed comedo and it will generally produce a white bump commonly referred to as a whitehead. As the sebum becomes exposed to the air it causes the black color to appear.  White and blackheads can remain in the skin for long periods of time.
